Membership Application This is form requires the basic information about yourself/spouse. Certification Form This form is stating that everything you stated is true & correct. Equal Access Form This form is where you choose a long distance provider, if you choose to have one. Special Features These 2 forms contain the special features you may choose for your phone. FCC Report This form is optional, but we ask that you fill out this information. Fed Gov Report This form is also optional, but we ask that you fill out this information for out.
